General Description:
The Transition of Care (TOC) Technician reports to the Manager of Mission Control or designee. The purpose of this position is to provide service to various departments by performing the necessary transportation of patients efficiently and expediently to the required destinations within the hospital. Under the direct supervision of the RN/LPN, provides in-patient/out-patient lifts, patient transfers, and patient movement. This position contributes to customer satisfaction by fostering collaboration and service excellence among all areas of the facility. The position shall exemplify the desired Culture of Choice® and philosophies of Parrish Healthcare.
Key Responsibilities:
- Transport patients in a safe, courteous and respectful manner to and from between nursing units, diagnostic areas, Emergency Department as appropriate to include neonates, pediatrics, adolescents, adult and geriatric patients utilizing all forms of transport to include: stretcher, wheelchair, Haustead recumbent chairs, patient beds.
- Appropriately uses lift equipment/aids for safe patient movement. Provide safe patient movement while assisting nursing staff with high frequency and problem prone lifts, turns and transfers.
- Transport patients’ chart to the department for procedure and returns chart with patient instructions to Nursing Unit as appropriate.
- Notifies staff of patients’ arrival and location within department and notifies staff of their return, ensuring all safety measures are in place (i.e., bed alarm, TABS, etc.). Alerts same of any changes in patients' condition. Communicates patient needs and/or requests to technical staff. Demonstrates professional attitude.
- Observes patients for unsafe behavior, uses healing conversations to re-direct patient with courtesy and respect.
- Assists technical staff in moving of patients to and from transport equipment and procedure tables.
- Communicate any transport delays to dispatch and maintain continuous contact with dispatch.
- Comply with all infection control and hazardous material measures at all times.
- Participate in all Customer Service Training in-service as provided by department.
- Care for equipment used by the central transport department; report defects promptly to supervisor.
- Clean and change linens on transport equipment as required.
- Maintains proper working order of portable oxygen canisters and reports to supervisor any faulty equipment.
- Performs similar or related duties as assigned.
- Knows fire, disaster and safety procedures and regulations as it pertains to the work area

Required Licenses, Certifications, Registrations:
- BCLS required within first three months of employment and must maintain and keep up to date during employment.
